To save time while at our office, we recommend that you download these forms in PDF format, fill them out, and bring them with you for your first appointment. If you do not have the opportunity to download these files, you can simply fill out identical forms found at our office.

Insurance will cover most procedures we offer. If there is a question about coverage, our staff will determine if your insurance will approve the expenses. If your insurance does not cover a procedure, payment options can be arranged to meet your individual needs. We accept all major insurance including:
